Residential Placements
Perthyn runs two Assessment and Transition Services in Wales. Here we provide specialist supported accommodation for people with learning disabilities, challenging behaviour and / or complex needs who desire to live in ordinary homes in the community. Subject to vacancies and admission criteria, people can be placed here by Local Authority and Health Authority Commissioners from any part of the Country.

The purpose of the Assessment and Transition unit is to enable individuals with challenging behaviour and / or complex needs and / or autism, to return to and reside successfully within reside within their locality. The placements do not provide a ‘home for life’ and as such are time-limited, ideally up to 2 years. During this time a comprehensive assessment, which can evidence individual need, will be completed. This will then be used to inform ‘move on’ to smaller, long-term accommodation reflective of individual need.
Referral to Specialist Support Services Referrals to the Assessment and Transition Units can be made by health and / or social service professionals who are seeking a placement within a specialist service provision. In order to process the referral an initial screening assessment is completed by Perthyn Specialist Support Service staff to determine the appropriateness of the referral / request for placement.
Individuals who are referred for service provision should meet the following admission criteria in the first instance.
- Adult with a learning disability
- Challenging behaviour
- People with Autism
- Complex needs / mental health difficulties
- Originate from the locality
- The individual requires assessment to evidence future service provision and support.
- Placement objectives are identified
- Specialist support provision required
- Referring agencies have a commitment to move-on proposals
- That the placement is short to medium term, i.e. up to 2 years
