Perthyn is committed to supporting people to live a valued life in the community and have real expectations for everyone we support.

We work in partnership with the person, their family and friends and the MDT,  to design individualised support that can flexibly meet a person’s needs and identified outcomes.

We will start by establishing the individual’s current needs, skills, abilities and opportunities. Alongside the information from the commissioners we carry out further detailed assessments of skills e.g. communication, mental capacity, financial, personal care and domestic, social, keeping healthy and safe, physical ability and risks, agreeing what they can currently do for themselves, can do with support or needs someone to do for them.

We help people to design their service by facilitating a Person Centred Review and using tools and methods best suited to them, encouraging them to look at the whole of their life, their dreams and aspirations and opportunities for skill development. When identifying changes and improvements, clear goals and actions are recorded.

Action Plan is then developed with the individual, to help ensure clarity about the outcomes that the individual needs support to achieve and steps agreed to achieve them. It identifies the areas the individual requires support with and resources will be targeted to deliver this. It acts a signpost to more detailed information e.g. personal care plan, eating and drinking plan, positive behaviour support plan and risk assessments

Where required, we support people to develop communication methods and skills to help them increase the opportunities to feedback their own views.
