We use the Active Support Model as a framework to further plan and target support to the individual and to measure the outcomes achieved. It promotes independence by skill development, participation, positive engagement, relationship building and community integration. We use Activity and Support Planners to identify which staff are supporting each individual with each activity, ensuring they have the right skills and attributes for each task.
These provide clarity for staff on what they need to prepare to support people with in advance, to maximise success. These are also provided in a variety of formats for the individuals supported – by photo, objects of reference and interactive technology for example. Detailed plans support staff to deliver consistent and regular support, indicating how often an individual has planned a particular task or activity to ensure they can practice and build it into their routine, the level of support required; from a verbal prompt to physical, ‘hand on hand’ support and breaking down activity into smaller steps to aid learning.
